 你缺碘了嗎？ 快速檢驗一「碘」都不能少

【健康醫療網／記者林怡亭報導】碘是製造甲狀腺素的必要元素，碘元素缺乏在人生的每個階段都會造成影響。孕期中缺乏碘會影響母體與胎兒的甲狀腺素製造，可能增加流產機會、影響胎兒神經系統發展及未來智能；新生兒時期碘不足可造成甲狀腺功能低下、智能不足；在兒童與青少年則可能導致甲狀腺腫大、甲狀腺功能低下、影響生長發育智能發展；而成人時期的碘不足可造成甲狀腺腫大、甲狀腺功能低下、影響心智功能、及碘誘導甲亢症等。 

兩種碘檢測法 ICP-MS為黃金標準

碘的檢測方式，按世界衛生組織的建議，包括以分光光度計的比色法，及感應耦合電漿質譜儀(Inductively coupled plasma mass spectrometry簡稱ICP-MS)。分光光度計的比色法因不須高階實驗室設備，是目前全球最多實驗室使用之方式；大部分實驗室經由加入美國疾病管制局(CDC)的Ensuring the Quality of Urinary Iodine Procedures (EQUIP)計畫或標準品之檢測可得良好準確性。至於感應耦合電漿質譜儀，是碘檢測之黃金標準(gold standard)，本身具有高度正確性與靈敏度，可檢測低濃度的樣品。

台灣醫師合作開發 檢測分析孕婦碘營養狀況

臺北榮總新陳代謝科黃君睿主治醫師、臺北市立聯合醫院陽明院區新陳代謝科王繁棻主治醫師、臺北榮總臨床毒物與職業醫學科共同合作，創國內醫院之先例，開發透過ICP-MS檢測之方式來進行有關孕婦的碘營養研究分析，以瞭解國內懷孕婦女的碘營養狀況。未來也將逐步瞭解其他容易缺碘族群的碘營養狀況，以促進全體國民健康。

美國疾管局認證 尿液檢測只要3分鐘

ICP-MS為高規格的分析檢驗儀器，須由專門技術員操作，因此多半僅在各國大學的研究中心或國家級研究單位使用。臺北榮總臨床毒物與職業醫學科的ICP-MS除可檢測多項微量金屬元素(如鉛、汞、砷、鎘)外，現階段發展尿碘的檢測技術已加入美國EQUIP program。檢測結果更已達CDC容許範圍且與標準值十分接近，每支尿液檢測時間只需3 分鐘，且靈敏度優於傳統比色法。
此外本檢驗方法的檢體前處理採用氨水，比以強鹼且具腐蝕性的化學物質「氫氧化四甲基銨(TMAH)」的前處理方式，在使用上更為安全，也更符合綠化學(green chemistry)的時代潮流。
